Application for Membership

Unfortunately, we've had to disable on-line applications due to a large amount of incompleted paperwork from this process. We currently don't have the time or staff to "chase after" people who apply and pay electronically, then don't send in their required paperwork to complete the process.

In it's place, we are providing this printable form. All new applicants must

  1. print the form, sign, and mail it to us with payment
  2. include a Xerox copy of the certificate of completion from their SI training
Failure to complete these steps will delay an application's processing.

If you did not attend any of the schools of structural integration which we've evaluated to fulfill our entrance requirements, you can still apply using the hardcopy method. Just be sure to include documentation of the dates, hours, and instructor contact information for the training you've received so the application committee can make a determination on your eligibility.

Renewing members can still use the members-only on-line renewal form and pay electronically. Your username and password required — email us if you don't know yours.

The application fee is in US dollars. If the applicant pays with a credit card, the card will automatically be charged the correct fee. Checks drawn on foreign banks must be in US dollars and take much longer to clear and process.

If credit-card information and email address are provided on the form, we will send a confirming email when the card is charged. A registration packet will be mailed shortly after that to all new members.

We currently cannot process applications any other way and apologize for any inconvenience this may cause.
